An accurate diagnosis, only possible by CT scan, is essential in these fractures. The depression of the central fragment must be precisely reduced to the talar dome and the resulting area of bone loss substituted by cancellous bone graft. The choice of the implant will depend on the plane of the fracture line, frontal or |
sagittal: cloverleaf plates on the medial aspect of the pilon, of which one leaf can be cut for a better adaptation when necessary, or tubular plates on its anterior aspect. The plates will have a buttress function and their epiphyseal screws will reinforce the bone graft. |
