These are fractures of the proximal tibia involving the articular surface of one tibial plateau. The subgroup .1 involves the lateral plateau (the .2 the medial plateau) with a vertical split fracture line. The subgroup .3 has an oblique fracture line involving the tibial spines and one of the articular surfaces. Ref. Manual of Internal Fixation:
208 - 213. 568 - 576.
Surgeon. RO, RO, RO.

Anatomic reduction is mandatory. The small fragments from the rim of the plateau can be fixed with 4 mm cancellous screws. The large fragments require 6.5 mm screws. They must be placed through a proportionate size plate to provide a buttress effect and assistance in the interfragmentary compression. The washers should be used only in the most proximal epiphyseal area and when a buttress plate is not necessary.

In the subgroup .3, the third fragment (tibial spines) must be reduced and fixation is achieved by the interfragmentary compression between both condyles or it may require an independent lag screw.
