| These are fractures that involve the articular surface of the femoral head. Therefore, they are intra-articular and disturb both the blood supply and the articular surface congruity. They are usually associated with a hip joint dislocation. | Ref.

Depending on the size of the fragment and the general condition of the patient, the possibilities are: excision of the fragment, anatomic open reduction, with or without internal fixation, hip joint arthrodesis and partial or total joint replacement. |
CT scan is mandatory for the essential accurate diagnosis. The small fragments of the inferior pole of the femoral head are best treated by excision. The recommended treatment for the large fragments is anatomic reduction and internal fixation with small fragments cancellous screws, as shown in the fracture 31-C1.3. |
