The B3.1 and B3.2 are fractures of both bones of the forearm combining a simple fracture of one of them with a wedge fracture (fragmented or not) of the other bone. In the B3.3, both bones have a wedge third fragment. Ref. Manual of Internal Fixation:

When one of the bones has a simple fracture, that bone must be reduced and fixed in the first place because this will make easier the reduction of the bone with a third fragment. In the B3.3 cases, the easier bone, usually the ulna, must be reduced first.

 

The surgical technique is the same that has been described for the isolated fractures of each bone. If there is any bone loss, even if it is minimal, autologous cancellous bone graft must be added, taking care not to place it in the interosseous membrane.
