| These are complete articular fractures of the distal humerus, involving the articular surface and the metaphysis with simple fracture lines, with slight displacement (C1.1) or with marked displacement (C1.2). The C1.3 subgroup corresponds to a T-shaped fracture with the horizontal fracture line through the epiphysis. | Ref.

These fractures require anatomic reduction of the articular surface. Both articular fragments are fixed with lag screws and to the metaphyseal area either with screws or with contoured reconstruction plates placed in the lateral borders or in the posterior aspect of the distal humerus. |
In the C1.1 case, the intra-articular fracture was reduced with a bone clamp and fixed stably with Kirschner wires. Following placement of the ascending screws, a solid fixation of the whole assembly was obtained and it was thought unnecessary to place a horizontal cancellous screw. Had the cannulated screws been available, it would have been very easy to insert one over the Kirschner wire. |
