These are extra-articular fractures of the distal humerus metaphyseal, with a transverse fracture line. Ref. Manual of Internal Fixation:
184 - 187. 446 - 449.

These fractures are frequent in children but rare in adults. They can be fixed with two 4.5 mm. self-tapping cancellous screws that usually get a good hold in the inner walls of the quite compact metaphyseal columns. The screws should not invade the olecranon fossa. The smaller diameter (4.0 mm.) cancellous screws usually break if used in isolated fashion; This is why it is preferable to insert two parallel screws (13-C1.2) when

 

using this type of screws. The osteotomy of the olecranon can usually be avoided.

When using plates, they should be properly contoured and placed on either the posterolateral or medial aspect of the bone. We prefer to use the reconstruction plates, which can be easily contoured, allowing a perfect adaptation to the bone surface.
